Some I can think of:
Judge Frollo (Hunchback of Notre Dame) Carrie's mother Sunlight Gardner (The Talisman, soon to be a movie) The people in Jesus Camp (sadly, not fiction)
and last but definitely not least:
Mrs. Carmody (The Mist)

Don't forget Footloose.
What was interesting is that the fundie preacher here actually begins to realize that by being fundie he's fucked up the entire town and by the end of the film shows signs of wanting to try and make things right.

Two leap to the surface of my mind: Inqisitor Bernardo Gui (F. Murray Abraham) and Jorge de Burgos (Feodor Chaliapin Jr.), both in "The Name of the Rose". Wonderful movie, and featuring two of my favorite actors, Sean Connery and Ron Perlman. Edit: The book is still slightly better.

Just to show the quality of my thoughts, I'm gonna go ahead and mention Sunstorm here... even if he's not a fundie of the Christian variety... The Commander from Handmaid's Tale... or, well, most of the people in that.

Just thought of something.
Did Jack Chick put in an appearance in that movie he helped sponsor?
If so, he'd count too.

Jedi Knight beat me to Burgos Half the characters in Aquasilva are fundies, Sarhaddon and Lachazzar most predominantly, but that's not exactly notable.

The pardoner from the Canterbury Tales. He was a twisted motherfucker (with the best story out of all the characters).

Gerry Preston from Waterloo Road - TV, but worth a mention. Appears in the final episode of season 2 as the guy trying to buy the school, in a ripped from the headlines story about fundies trying to gain influence in the education system. Initially gets dismissed as an eccentric (which is how fundies do tend to be seen in the mainstream in the UK, because despite their best efforts they rarely get enough influence to be dangerous) but they discover he's more sinister than that. At the end of the episode he's giving a sermon where he says that a teacher who committed suicide in the previous episode could have been saved if God had given her strength, at which the deputy head stands up and delivers a right royal verbal smackdown which I unfortunately can't find on YouTube.

As was stated before, the penguin leader from Happy Feet.
Most of the characters from The 19th Wife
Ned Flanders from the Simpsons
I'm sure there's more, I just can't think of them off the top of my head.

Robert Duvall as Frank Burns in M*A*S*H. (the movie)
F. Murray Abraham as Antonio Salieri in Amadeus. (before he decides to try and destroy Mozart)
Debating in my own mind: Laurence Fishburne as Morpheus in The Matrix. (and sequels)
